• Employer group plans currently offered nationally to self-funded companies
• Employer group plans for fully-insured businesses to be offered beginning Oct. 1, 2004 in states providing regulatory approval (AZ, CO, GA, KS, LA, MS, UT)
• Employer group plans for fully-insured businesses to be added to additional states as regulatory approval is secured
