Can I pay for cosmetic surgeries with an HSA?

No. Surgeries performed solely to improve someone's appearance are not deductible as a medical expense. As the Internal Revenue Service states in its guide to Medical and Dental Expenses, "You generally cannot include in medical expenses the amount you pay for procedures such as face lifts, hair transplants, hair removal (electrolysis) and liposuction."
